The deadline for your benefits enrollment for 2012 is 12/26/2011

Please find below a link to an Enrollment Worksheet to help you prepare for making your benefits elections.

You MUST complete the enrollment process even if you do not plan to participate in the benefits programs.

Click on the doctor to go your Log-In page.

 At the Log-In Page:

• NEW USERS - click the NEW USER button and provide security information as prompted to create your own confidential Log-In.
• RETURNING USERS - you will be prompted to eneter your username and password

Forgot your Username and/or Password?

• You will be asked to verify your SSN, DOB, HINT QUESTION and ANSWER in order to acces the system.
• If you are still experiencing problems, please contact your HR Department immediately

Click on the ENROLLMENT button to begin your process.

At the end of your enrollment, remember to follow any ADDITIONAL instructions carefully as you may need to submit signed enrollment forms to your HR Department.
